The enclosed nomination form must be completed and, along with any other supporting documentation, be received by the Chairman of the Nominations Board no later than October 15th before the year the nominee will be considered for induction to the HOF. The nomination may be sent directly to the Chairman, any Nominations Board members, or the museum. The last page of this nomination form contains the names and addresses of the Board members and the Museum. The Nominations Board will hold a deciding vote on each nomination by February 1st of the year that the nominee will be considered for induction to the HOF.
Any documented nominee who is not enshrined the year their nomination is considered will be automatically carried over for consideration the following year. If not selected that year, the nomination will be returned to the person who submitted the letter.
